Patient Information

Everything you need to know before your visit

Building a relationship based on trust matters.

We focus on understanding how you feel and helping you stay comfortable during treatment, especially if you experience dental anxiety.

New patients

We love meeting new patients and promise to make your first visit as comfortable as possible. Your first appointment typically includes a comprehensive exam, X-rays, and a cleaning — giving us a complete picture of your oral health so we can build a care plan together.

Please try to arrive a few minutes early. Here's what to bring:

  • Health card (OHIP)

    Required for all appointments

  • List of current medications

    Including vitamins and supplements

  • Previous dental records

If available — not required for your first visit

  • Insurance information

Benefit card or plan details, if applicable

Preparing for your visit

Feeling nervous before a dental visit is completely normal. Here are a few things that might help:

  • Arrive 10 minutes early

This gives you time to settle in, fill out any paperwork, and ask questions before we get started.

  • Let us know if you're nervous

Dental anxiety is more common than you think, and there's no shame in it. Tell us before or during your visit and we'll adjust our approach.

  • We go at your pace

You're always in control. We can pause at any time, explain what we're doing, or take a break — just say the word.

  • Ask all your questions

No question is too small. We want you to feel informed and comfortable, so please ask us anything that's on your mind.

Patient forms

New patient forms are available at the clinic — we'll have everything ready for you on arrival. No need to print or fill anything out beforehand.
